Minor Rental Agreement

1. A parent must make reservations. Said parent will provide a contact phone number in case of emergency and appoint a group leader to communicate with the driver.

2. Extreme Limousine requires a non-refundable deposit of $400 to hold your reservation date, and full payment must be received 14 days prior to pickup date. ALL PAYMENTS ARE NON-REFUNDABLE.

3. THERE IS ABSOLUTELY NO SMOKING , DRUGS OR ALCOHOL. A parent or guardian will be contacted if this is violated.

4. The privacy window will be locked in the OPEN position.

5. Personal baggage such as backpacks and large purses will not be allowed in the interior passenger compartment.

6. All passengers and all signors of this agreement shall be jointly and severely responsible for any damages done to the vehicle as a result of the intentional, reckless or negligent conduct of any passenger. At the time you board our limousine, you must bring to the attention of the driver any damages you may notice in the interior of the passenger compartment.

7. Extraordinary clean up (including but not limited to vomiting) will be assessed at $250 per occurrence.) Broken glassware will be replaced @ $15 per piece.

8. Extreme Limousine reserves the right to remove any person from the vehicle and/or terminate this contract in the event of a violation of any of these special conditions by any passenger. In the event of such early termination, no portion of the contract prices shall be refunded.

9. The sunroof must be locked closed. It is an emergency exit only. If it is opened at any time, the ride will come to an immediate end and no money will be refunded.

In the event that any part of this contract is declared unlawful or unenforceable, the remaining provisions of this agreement shall remain in full force and effect.

I/We have read and agree that all information regarding trip information has been furnished by me and is accurate. I have read and agree to abide by all terms of this agreement.

Rental Agreement

1. Extreme Limousine requires a non-refundable deposit of $300 to hold your reservation date. Full payment must be received 14 days prior to pickup date. EXTREME LIMOUSINE REQUIRES ALL PAYMENT IN ADVANCE.

2. CANCELLATION: ALL PAYMENTS ARE NON-REFUNDABLE.

3. Extreme Limousine shall not be required to transport more than 6-20 passengers in said vehicle.

4. All passengers and all signors of this agreement shall be jointly and severely responsible for any damages done to the vehicle as a result of the intentional, reckless or negligent conduct of any passenger. At the time you board our limousine you must bring to the attention of the driver any damages you may notice in the interior of the passenger compartment. Other than damage so called to the attention of the driver, you will be held liable for damage or extraordinary clean up expenses caused or incurred with respect to the interior of the passenger compartment.

6. Passengers who are not of legal drinking age may not possess any alcoholic beverages or have any personal baggage in the interior passenger compartment.

7. THERE IS ABSOLUTELY NO SMOKING ALLOWED IN THE PASSENGER COMPARTMENT. PLEASE ASK THE DRIVER TO STOP IF SOMEONE IN YOUR PARTY WISHES TO SMOKE.
